I’m extremely happy to know that you value your health and are seeking an integrative approach to improving it and I’m extremely privileged that you’re allowing me and the rest of the Focus team to come alongside you as you take this journey of optimal wellness.

For the next several minutes, we’ll be taking care of some prerequisites. I estimate that it will take 15-30 minutes, so if you don’t have the time right now, feel free to come back later and complete the process. We need the entire process to be completed at least 48 hours prior to the start of IMH Bootcamp. Also, please keep in mind that we will never sell or share your information without your written permission.

Here’s what we’ll be doing:

• When you click the button below, you’ll be taken to our on-boarding intake and survey, where you’ll provide us with information about your health, and our program participant agreement which explains our policies.
• After you complete, sign, and submit the forms, you’ll be redirected to our website where you’ll see our team member introduction page. There you’ll be able to learn a bit more about the Focus team members you’ll be interacting with during this group program.
• After being introduced to the team members, I’ll take you through a brief orientation to show you how to find and navigate through the various program modules and other content areas on the site.

After you finish these important pieces, you’ll be all set and ready to go.
